The best-loved event for the people of Montepulciano is the Bravìo delle Botti, a challenge among the eight town districts which is held yearly on the last Sunday in August as a tribute to St John, the local patron saint. Formerly a horse race, the Bravìo was revived in 1974 as an exciting uphill run with teams of two “pushers” for each participating district. The team's task is to roll an 80-kilo wooden wine barrels for about 1.8 kilometres to the finishing line in Piazza Grande.

The Compagnia Popolare del Bruscello Poliziano stages since 1939 one of the most important forms of Italian folk theatre. The Bruscello is a unique theatre genre in which the story – usually an epic or folk tale – is sung in interlocking octaves by amateur performers. In times past, it was highly popular in the Tuscan countryside. Today, a few shows are held in mid August in front of the Cathedral, in Piazza Grande.

The Cantiere Internazionale d'Arte Foundation organizes in July a renowned festival featuring opera, concerts and theatre. It also offers music classes and manages the programme of the 18th C. Teatro Poliziano, running from December to March with top-notch shows.

From April to October, Palazzo Ricci hosts a concert season run by the Accademia Europea di Musica e Arte. At the beginning of September, the programme highlight is an unmissable International Week of Chamber Music which brings together top instrumentalists and the most promising pupils from the best music schools worldwide.

During the other half of the year, from October to April, the historic Caffè Poliziano offers a Saturday programme of literary meetings called 'Aperitivo con autore', with afternoon readings and music.

The City Museum Pinacoteca Crociani hosts important art exhibits and takes part in events such as Amico Museo, Notti dell'Archeologia and the international Nuit des Musées.

The many events celebrating the glorius local wine and food include Cantine Aperte (late May), Mercoledì del Nobile (June–July) and Calici di Stelle (August 10th). In February, Wine experts and dealers from around the world cannot miss the Anteprima del Vino Nobile event, an advance tasting of the new vintage.

Markets and fairs include a monthly Arts and Antiques Fair (every second Sunday of the month and preceding Saturday), the Olive Oil Festival in December and the Mostra interprovinciale dell'Artigianato over the Christmas holidays.

The most important events held in the surrounding villages are the Fiera di Primavera at Montepulciano Stazione (end of May), the Palio dei Carretti at Valiano (fourth Sunday in September) and the Live Rock Festival at Acquaviva (late August – early September).
